Privacy Policy

Smile On Purpose, LLC uses the information gathered solely to assist you during your sessions.

Smile On Purpose, LLC is not a licensed clinic and cannot medically diagnose a client nor can we prescribe medication.

Smile On Purpose, LLC has trained, certified, Life Coaches and will never display or advertise as a licensed Therapist. 

Client/Coach Agreement

Clients are not legally bonded to any contractual agreements. Therefore, clients have the right to request for copies of session notes or any other document pertaining to the sessions; however, clients cannot receive original documents. Original documents will remain secured with the assigned Life Coach, as well as the owner of Smile On Purpose, LLC, the Lead Life Coach. 

A client agreement is an understanding of the services rendered and rights of privacy. Each session and each document will remain confidential, unless the client signs rights for another party to review documents pertaining to the sessions.

A Coach's agreement is an understanding of the clients needs. A Coach will never force a client to do anything they do not desire to do, but a Coach will encourage the client to step out of their comfort zone. A Coach will not push their clients, but a Coach will lead their clients.

Minors

Anyone under the age of eighteen has privacy rights as well. A Coach will never hide anything from a parent/guardian; however, if a minor requests that certain things are not shared with the parent/guardian, we will like to maintain that rapport with the child. If a parent/guardian requests for session specifics, we will discuss the session, with the minor present. Prior to the session, the assigned Coach will have a separate consultation with the parent. 

Rights Revoked

A clients rights will be revoked, if the assigned Life Coach determines that the client is in danger of themselves or others. In this case, the proper agency will be contacted to ensure the client receives the necessary care. Before releasing information or contacting any agencies, the owner and client will have a session to determine if additional help is required and to explain the next steps.

Recorded Sessions

If a session is recorded, it will be to fulfill any session obligations to better assist the client. The client will be fully aware of the recording and must state that they are aware of the recording, at the start of the recording. The recorded sessions will remain secured and never shared.

Protected Information

Though we are not licensed health professionals, we promise to follow HIPPA, to the best of our ability. 

Your full name, date of birth, address, method of payment, and appointments are all secured, but will be accessed by the employees of Smile On Purpose, LLC, only.

Smile On Purpose, LLC promises to never sell your information.

Smile On Purpose, LLC agrees to contact you personally and urgently, if there is ever a third party requesting for your information.

Confidentiality 

We take pride in confidentiality and promises to never speak, write, record, or send any information to anyone regarding your sessions, without your written or recorded consent.

Feedback

Positive feedback from the client may be posted in Testimonials on Smile On Purpose, LLC's website, www.ismileonpurpose.com. The client's initials, City/State, followed by their feedback, will be displayed, but never the details of the session.
